The Health Status Questionnaire 2.0, or HSQ, is a method for measuring health attributes, health status change, and risk of depression. Eight-subscale is used as the structure of the test. This test is used as a close associate for the Short Form Health Survey in creating the OMS (Outcomes Management System) anticipated national database of outcomes information.
The HSQ is intended for ages of 14 or older. There are 39-items which are answered with multiple choice and the test can be taken online or with paper and pencil. Approximately 5 to 10 minutes are required to complete the test. The 2nd edition (HSQ 2.0) is the current version of the questionnaire.
